Yesssss, it’s cursed and can’t be put away or dropped until it’s tasted blood. Also, seconded cutting heads off but only if the target is out of limbs.

cursed sounds like his grace all over again. maybe make a cursed variant if you want, but the default isnt cursed

Temper it with soulsteel to trap the ghosts of the people you kill inside it, hivemind style.

Does spinning or twirling the item sometimes fail and cause you to lop off an arm or leg? 

Because it should.

Would be neat if the sword has a clean and a bloody state, and a twirl would fling blood off similar to a throat slash suicide and clean the the blade.

Also a part of me think the sheathe should also be a weapon, for non-lethally bonking people
