I don't see why obesity SHOULDN'T be a thing, it also should come with a plethora of random horrific conditions, including but not limited to: Heart failure, stroke, low stamina regen/low stamina, skin necrosis (very rare), flatulence, and diabetes (actual chronic disease that gradually ups your sugar level).
