Certain commands do nothing

While playing, under the commands menu, certain commands do nothing.

Equip, Hotkeys, Help, and Whisper in particular.

By nothing I mean it is as though I've not even clicked on it at all, there is no reaction, no sound, no visual cue, nothing.

Equip, Hotkeys, Help, and Whisper in particular.

Equip only works if you hold a item you can equip.
Whisper only works when you are active and playing.

What does the hotkeys window look like; maybe I could somehow figure out what's wrong by seeing that?
