The general idea for this implant is that it's a medical use one that works with cloning record implants to monitor someone. The difference is that if it detects a death event, it automatically teleports that user's corpse to the cloning room. It doesn't work if the user doesn't have a cloning implant, or is gibbed.
Really, the use of this is to recover more tangible things from someone's body. Gear, implants, augments, etc. That way, being record-cloned doesn't mean you get stuck nude in the hallway with no access to your old body!
I still haven't decided how this implant would be obtained. Maybe it's a hacked medifab thing, maybe it's a D.O.C sale item, maybe it's a cargo order, I dunno.

It'd have to be rare as this could be bad for a few antags
Namely traitor, changeling, and vamp, as it could immediately out all of them. Traitor can't loot whatever they were going for off the person they just killed (such as the captain for their gun or ID) and changeling made husks would get teleported to cloning and immediately out said changeling before they can strip them of their gear to hide their identity, meaning they can't use that person now cause everyone knows they got eaten by a changeling. For Vamps, in order to make ghouls the target must be dead, but if it gets insta teleported away they can't do that and they get outted since the target likely has no blood.
Combine this with forensics and finding the culprit of these murders would be made painfully simple as finding the body wouldn't need to happen anymore, ultimately killing stealth. Theres probably some more points here, but I can imagine a teleporting death implant would be hell for antags.

I have to agree with Badshot about how it would change the dynamic of corpses and antags.
Some other points to consider:
- The recall artifact is this but worse because activation isn't forced. Is that okay, would you change anything in regards to recall artifacts?
- To some degree, Telesci does this but worse (because it requires a person to operate and do it at least a little manually, not in an automatic fashion)
- Have you considered how easily this creates a "Snatch-and-grab-suicide" situation where you can retrieve items that can be stored on a body, perish, and bring them to a safe space with 0 consequence? Not saying that's a deal breaker, but that's a heck of a tactic I'd want to consider pretty carefully while implementing something like this.
- Flipping it on its head, my first conniving thought on this idea is to insert something fatally explosive into a person before they die, then trigger the device upon arrival, delivering a bomb, something I'm only usually achieving with a conspicuous beacon or telescience.
However, I'm not necessarily saying it's a good or bad idea, just that it's an idea that seems to encroach on a lot of other parts of the game, and depending on how easy it was to obtain would supersede them quite significantly.

Might be a good high-point salvager reward, and since salvagers already have a teleportation device, it would act as a direct upgrade in a way.
I don't think this should be a item the crew has access to though, dying should be a debilitating thing, and in that sense it's a good thing you don't have anything at all after cloning.

Iirc, technically you can already do this with tracking implant + health implant and packet/dwaine things.

This makes my habitual need to create messy evidence easier too, the idea of setting the implant to the front desk of security and stapling a note to the body does appeal.

Oh yea.. Imagine insted of cloning bay.. it's ANY TELEPORT BEACON... wich mean it leaves evidence regardless (but now you prep to get rid of a body)
But you can also just set it to teleport it to the brig with a note.... Now we are cooking finch!
