Should be minor, more of a cute detail than a notable downside; I'd say like a 20% chance for 1 damage to one of the kidneys per life circle.

I mean, it is kinda nice touch. Then again, energy drink is not often used because if its heart-stopping drawback (and meth existing). It would be a nerf for flavor reasons, which doesn't sit well usually.

Honestly I could well see energy drink being rebalanced by just upping the caffeine intake it causes and letting THAT handle the eventual heart failure instead, and giving it some minor kidney damage to compensate.
