Minor Vending machines and crates have items appear under them

This includes stuff like the nanomed. I noticed it first on Oshean, but it might be in other places too.

This bug appeared from BYOND updates, I'm pretty sure. We can fix it though, someone just has to go through and manually set the layer of dispensed items for vending machines (and possibly have crates set their own layer / the layer of their contents if that's not too costly)

setting layer isn't costly at all, as long as it's all done together

appearance changes are batched together and sent at once to clients, unless you sleep between changing appearances, which splits up the batching and makes the whole thing much more expensive
