This has happened to me about two or three times, the Butcher's Knife will seem to just disappear after using it for a bit. I am 100% sure I'm not just losing it, especially since this has happened multiple times.
Most recent example, lately I did a simple 'throw knife at captain, grab knife, cut to death', everything went pretty well. He died horribly and I ran off into the hallway with knife clearly in hand, and when I went to put it in my backpack, it wasn't in my hand anymore. It wasn't anywhere in the path I came from, it wasn't anywhere my backpack or person, and there was no message indicating anything, it just vanished.
