Maybe it would be a toned down, subtle effect. Like, very faint color shifts, the hallucinated mobs just wandering around minding their own business, that sort of thing. Maybe other people look different, something kinda nifty but harmless.

Maybe add in a few critters and decorations for people with this second-sight trait.
