If it keeps happening I'll try and get more details for a bug report, but I'm hoping this was just a funny bug. I irreversibly mutated after a radstorm started and I got caught in the centre of a blast. I was teleporting into windows, making weird sounds, changing colour, and randomly colouring the tile I stood on, even after an entire vendor of mutadone.

Did I forgot to mention spontaneously combusting? Because I was spontaneously combusting. In a 3 x 3 fireball that also ignited things around me:

Sounds like you got EXTREMELY unlucky and got smacked with Booster Gene Q. It makes a bunch of miscellaneous bad shit happen, probably just below Mutagenic Field as far as bad mutations to have goes.
