Alcohol medical reactions.
Alcohol and medicine don't mix well in real life, especially morphine.
Sure this might increase bad people's recipes, but it would also be super fun for antagonists.
For instance morphine + alcohol should knock you out for a long time, if not kill you depending on dosage.
This shouldn't really affect other drugs currently in use, considering charcoal is the only thing that has an alcohol reaction.
So really all that would be necessary would be making alcohol and morphine cause a long sleepy time or death depending on dosage.
