What if Poison Bullets work a different way? Have them contain the poison that only deals damage once it's fully decayed, based on how long it's been active for. Make the bullet slowly release the toxin into a person's bloodstream whenever it's inside them, at a rate just a little faster than it's decay rate. Now, you have a tough choice. You're bleeding out from the wound, but if it's a poison bullet, when the doctor removes it you're going to take a shitload of poison damage from being steadily filled with this stuff ever since the original shot.
It'd be a little more subtle, would require doctors to start checking a patient's bloodstream before operating, and would make getting that bullet out quick an immediate priority, since the longer the bullet is inside someone, the more damage it'll do when it comes out.
It could also lead to the awesome situation of a player walking around, bleeding but unable to remove the bullet because it's been in so long, they'll die if it gets taken out. Have the loose-cannon detective shoot a syndicate with his own bullet, and tell him that he has three minutes to give up his accomplices, before the poison builds up enough to where he's dead meat. Fun times!
Alternatively, just fill the bullets with that toxin mixture somebody posted on SA which kills people in 30 seconds flat.
