Oh shit, how could I forget this one as a medic main, because it's truly ancient.
Chest Cavity Implant Surgery:
"Only items of from size tiny to normal, from flashlights to boxes, can be implanted. Things like saxophones, pills, drinking glasses, and other things that already have an effect upon applying it to a player cannot be implanted. (Consider this less a feature and more a long-standing bug that's difficult to fix.) "
I don't think this is something that is easily fixed and being only a "minor" limitation of a working feature, just gives it a really low priority.
But man, so many possible shenanigans when the if becomes an is.
