As non-constructive as Berrik's post is, he's a point. A player gaining immortality is rare, a player getting Macho Touched is rarer, and a player who has been Macho Touched previously gaining immortality is even rarer. It's so rare that's it's pretty much never going to happen, and when it does, I don't see it being any worse than infinite golden statues from people using the reward and reviving over and over or some sort of bug similar to the cases of certain deaths bypassing Soulguard.
If it's really a concern, the medal reward could easily check for immortality conditions and prevent it from being claimed, given there's maybe a handful of immortality sources. Heck, there's even a global var for unkillable players in the 2016 source. It'd avoid the aforementioned bugs, infinite golden statues, and, in the case the chaplain contracts, any lore or whatever issues about breaking a supposedly unbreakable/unfavorable Faustian bargain.
