Except fake moustaches, gas masks and medical masks and so forth of course. There's a lot of cool face-covering things that no one ever wears because you can't also wear internals with them. Free fashion!

Basically this. Yes, the masks are awesome. Everyone would love to wear one. But if it was as easy as that, everyone WOULD where them, and we'd end up with a creepy station with a bunch of masked weirdos everywhere. If you want to make a fashion statement; to stand out from the crowd, you have to make sacrifices! I know this is the most unrealistic space simulator ever, but it's got to have some kind of logic! How can you where a mask, and a breathing mask? Two masks? You better have two heads.

The problem with masks not having internals isn't that players are just so gosh-darned bad at the game that they can't click four times without dying. The problem is that the things which are most likely to expose you to vacuum (I.E. explosions, being blasted through a window, getting stunned and tossed out an airlock, etc.) are also all things which knock you down and/or stun you, preventing you from grabbing or doing anything.

I'm personally of the opinion that masks and breath masks are fine as two different things, give me fashion or give me death, but it's a bit shit to argue "LOL git gud nub" as a reason not to do it.
