I someone loses their ass to rathan's secret, the surgery system still counts them as having an ass, forcing you to preform surgery to remove their non-existent ass to be able to reattatch their ass.

I think it's more along the lines of the game thinking it's both inside the player and has a physical location outside of them. Theres a similar problem with manually removing cyborg arms that I'd forgotten about where you have to crusher the arms in order for the game to realise they aren't installed anymore.
