I'm sure this has been posted a few times at least, but I'd like to say it anyway. Maybe my take on it will diverge a bit.
Organ customization in character creation:
Like how you can customize your limbs and eyes in character creation, some custom organs could be selected.
Brain:
Heart:
Lungs (either done separately or both together):

Brain:
- Default: Regular human brain.
- Cyber: Uses the AI brain (maybe forces the AI laws even if you're a human?)
- Synth: Uses the synthbrain (would mean that some non-kudzu living people could have synthbrains)
- None: No brain. All the effects of brain damage minus the forgetting how to breathe and dying. If this is taken you'd still be alive despite not having a brain. Things that cause brain damage still do, but don't matter until you get to the point where you would take damage/die. The surgery to remove your brain still kills you but since you don't have one no brain is ejected. Grants one trait point to compensate for the fact that brain transplants can't save you. Mostly a joke option.
Heart:
- Default and synth.
Lungs (either done separately or both together):
- Default: Regular human lungs.
- Synth: Synthlungs. Costs one perk point, because of the way they prevent carbon dioxide poisoning and produce oxygen.
- Plasmatoid: Plasmatoid lungs. Could replace the plasma lungs trait, so it's no longer mutually exclusive with things like having hair as a lizard, or being stinky. Gives one perk point per lung, so +2 if you take both, same as the plasma lungs trait. Would still give you a plasma tank to breathe from.
- None: If customized separately, taking only one lung could give a perk point, since you'd periodically have some breathing issues. Obviously you shouldn't be able to spawn in with no lungs.
Butt:
- No customization. Customizing one's butt would be too OP
- Default and synth, same as the others.
- Maybe this one could have a cyber option?
Stomach:
- Default and synth.
Intestines:
- Default and synth.
- Maybe could be packaged together with the stomach, like on a synthstomach plant.
Liver:
- Default and synth.
Pancreas:
- Default and synth.
Kidneys (as with lungs, either done together or separately):
- Default and synth.
- None: If done separately, only one can be removed, and if done together, taking the none option still only spawns you with one kidney rather than neither. Gives you extra starting cash, as you sold one of your kidneys.
Spleen:
- Default and synth.
Appendix:
- Default and synth.
Other notes:
- Very few have cyber options because people being able to spawn in with them would give roboticists even less to do than they already have. With the cyberbrain option, people aren't exactly coming in for new brains very often - it'd be just for flavor. And for the cyberbutt option, people don't usually want/need a butt transplant, and cyberbutts are usually only used to make buttbots in my experience.
- This could be expanded in a way that actually gives roboticists more to do - People could spawn in with "faulty" or "damaged" cyberorgans, which come with no benefits, significant drawbacks, and must be removed and repaired by a roboticist. The drawbacks should be significant enough that people won't just ignore them and will actually go in for repairs, but not fatal, so if there's not an available roboticist (or any roboticist at all), you don't just die.
- Since spawning in with synthorgans would be mostly for flavor, most shouldn't have a perk point cost.
- Unless specified otherwise, people shouldn't be able to just spawn in with missing organs.
