Description of incident:
Warden ██████ entered cell 087 at 21:50 with her baton and a toolbox. She shouted at inmate Sips-The-Mix, announcing an intent to punish her (see incident report k-288F). She then cuffed inmate Sips-The-Mix and dismantled her hands, removing the thumbs and forefingers from her cybernetic arms. Note that no such disciplinary action was authorized, and is not common procedure for this facility. Inmate Sips-The-Mix spat in Warden ██████'s face, who retaliated by stunning her and leaving her cuffed in her cell. Warden ██████ left the cell at 22:13, and was brought to the infirmary at 22:20 experiencing dizziness, nausea, weakness and vomiting blood. By 22:23, Warden ██████ was dead.
A close search of inmate Sips-The-Mix's cell uncovered a loose brick concealing a hidden cavity with a stash of modified glassware and a colony of amanita mushrooms, along with another unidentified fungus which has been sent to the laboratory for analysis. Close medical examination of inmate Sips-The-Mix revealed a small cavity in her cheek composed of scar tissue, with strong traces of the poison that killed Warden ██████. The scar tissue did not bind to styptic powder or synthflesh, and has been sealed with a suture as a temporary measure to prevent similar incidents in future.

It is the opinion of this overseer that inmate Sips-The-Mix is criminally insane, and has demonstrated she has no true intention to reform or atone for her crimes. I protest the motion to transfer her into the Decirprevo corrections facility for white collar crimes, this woman should be placed in maximum security at the very least. I know you're reading this, I implore you to do the right thing, have some integrity and stop caving to their demands. I don't care how much bank this lunatic is making for the weapons division.
